           Samutsakhon hospital was pass Hospital Accreditation since 2006 and Health Promoting Hospital since 2007.Average 2,500 out-patients  and 500 in-patients prescription per daywe are work hard together to reach for pharmaceutical care goal and final objective to patient safety.Our teamwork are 34 hospital pharmacists ,who were worktogether in 7 unit .There are hi-light of technology and innovation that use in Samutsakhon hospital  to achieve pharmaceutical goal ."           1.Out-patients unit

• drug board queue

           Drug board can manage out patients queue with quite atmosphere and smooth service line.When paients get Rx queue (blue color paper),they know that they will wait for the time that number will appear at the board.It mean that their drug are ready to dispense from pharmacist.

• automatic counting machine

           Automatic counter machine (MVF) was use in department of pharmacy since 2007 .This machine will print drug label ,then officer will scan with barcode for accurate drug type and amount .It is helf for reduce workload and prevent medication error.

• Pharmacy Jor Jae (PJJ)

           "May I help you?" our special officer (PJJ) will take care patients who wait for drug board especially elderly,handicapped people with service mind.

• prepacking machine

          Some medicine was often use.we will prepare before use and label with expire date to save time and prevent dispensing error because we have double check for collect amount.

• burmese translator

          Because of Non-Thai labour in Samutsakhon hospital.Major is Burmese,It is necessary to has Burmese transaltors for dispensing medicine with pharmacist. Now we have 2 male translators and 1 female translator for improve pharmaceutical service with humanity.

• counselling room (007)

          When pharmacists found patient who risk to have drug related problem.They can refer the patient to counselling room to collect that .

          To prevent adverse drug reaction by pharmacist evaluation  and counselling to the patients that effect to patients safety.Most frequency drug reaction will happen in group of NSAIDs, Antibiotics(Penicillins)

• special clinic ( HIV TB Asthma )

             One stop service with pharmaceutical care (Tuesday for asthma child,Thrusday for TB patients and Friday for HIV patients) .HIV clinic are monitor adherence by pill count calculation to ensure that patient follow their medical plan.

• chronic care center

          Chronic care center is newly unit  to make holistic health care for chronic disease patients by cooperating with doctor , nurse ,nutrient expert ,physiacl therapist.

          3.In-patients unit

           This machice make medical envelop for one day dose system.Each patients can identify their name in each envelop that separate for morning , noon,evening and bed time.

            This label are contain important drug information such as stability , IV incompatibility ,adverse drug reaction monitoring for special drug (Norepinephrine,Phenytoin,Ciprofloxacin,Imipenam)It is use for knowledge sharing betweem nurse and pharmacists.

• chemo one stop service

           One stop service for patients who need for chemotherary was built since 2007.Multi-dispensary teamwork for overall care process by counselling and monitoring by nurse and evalauting and preparing by pharmacist to effective treatment for each patient      

           4.Acute care unit

• round ward

           Clinical pharmacist follow with doctor to visit patient in medical ward for working with pharmaceutical care process (ADR,Counselling,DIS). 

• screening drug profile

           Screening copy of doctor order sheet and drug profile to found drug related problem for other ward that without pharmacy round ward.Direct intervention with ward by telephone to prevent adverse drug reaction or medication error.

• trigger tool (Scr)

          With cooperation between labolatory and pharmacy,the report of patients who has abnormal functional of kidney (Scr > 1.3) will send  for follow up pharmacist for dose adjustment.

• drug use evaluation

           Pilot project for few medicine (Moxifloxacin,CeftazidimeErtapenam,Colistin) pharmacist will collect data for monitor the medicine that risk to drug resistance and inpropiate use in Samutsakhon.P

• minimum standard

           Standard guideline to prevent drug-drug interaction , tablet can not feed for the same standard with other pharmacist.

• Acute acre mobile team 

         Targeted patient who need counselling (Warfarin Asthma TB HIV DM)  Pharmacist will visit them with special counselling in their ward .To make sure that all In patient will take care by  our pharmacist team

• herbal medicine product

            To promote Thai traditional medicine use with local herbal product from samutsakhon hospital such as anti cough solution  , carrot gel, cucumber gel.

            New innovation to prepare extemperaneous preparation with convinence method for often use mediceine such as norfloxacin suspension.Pharmacist prepare medicine powder seperate with suitable vehicle .Only mix them together for use within time that label on brown bottle.

         To prevent expired medicine by using INV program for check first in-first out .we set standard operation procedue for manage medicine in stock room.

          Controlling temperature system start from manufactor to department of pharmacy until patients home with ice package to keep standard of medicine(insulin preparation).  

• medical bargaining committee

         Bargaining commitee from other hospital and provicence health office to prove and choose medicine in suitable cost and standard quality. 

          7.Supportive team

• Time table rotation   

          Multiple job time table for supportive team to help each unit in rush hours(Example : Out patient department in the morning or In patient department in the evening).Our officer can do many work , not specific.This strategy will buid learning by doing organization in the future.

• Pharmacist assistant training

          New official was trained by pharmacist and older officer in theory and practice.They must pass the examination and testing from every unit. To certificate quality of our  new officer by sharing experience together.
